Get Your Dependent Verification Information to Health Service System (HSS) by
this Friday June 15th, 2018.
San Francisco’s Health Service System (HSS) is having an Audit to find out if your Dependent spouse or domestic partner is still eligible for health care benefits.
They sent letters to those of us with Dependent spouses and Dependent domestic partners who receive benefits through CCSF.
We have now spoken with HSS directly and expressed our consternation with the letters.
We told HSS that our central concern was making sure no who should be covered would be dropped from their benefits.
HSS assures us that if there are any questions or concerns that you can reach them directly.
There will also be an appeal process for anyone who believes they have been cut off of benefits unfairly.
If you do get a notice of cancellation of your benefits for your Dependent spouse or Dependent domestic partner contact our union ASAP at 415 585-2121.
Please make sure that you provide the information that HSS is requesting by this Friday June 15th, 2018. If you supply a tax return, you may blackout all of the information on the return except for the Form 1040 heading and your name and your dependents. If providing utility or other bills/statements, block out account numbers.
